What we do

Connecting support around people after diagnosis.

NexaGen helps people living with long-term conditions find trusted support after diagnosis and between appointments.

We connect healthcare, GP practices, charities, community organisations, care partners and practical support around the person.

The support often exists.

The problem is that people and families are left to find it, understand it and join it up on their own.

NexaGen helps make that support easier to find, easier to trust and easier to use.

We are starting with Parkinson’s and neurology, with a model designed to support wider long-term condition care over time.

NexaGen helps connect:

  • healthcare and GP practices

  • trusted charities

  • local and community support

  • care partner guidance

  • practical next steps

  • digital tools where helpful

  • condition-aware services

  • research-aware information where appropriate

We do not replace the NHS, charities or local services.

We help them connect around the person.

Why this matters

People living with long-term conditions often need more than clinical care.

They may need trusted information, emotional support, practical advice, family guidance, local help, peer support and clearer next steps.

Many of these services already exist.

But people are too often left to find them on their own.

NexaGen helps create a clearer route to support.
